I understand the difference.
I look at it this way. A medical professional(nurse, whatever) has a list of tasks. Lets assume they perform each of those tasks equally well for all patients, as they should.
The difference? Maybe you go the extra mile to make sure Mrs soandso is comfortable, listen for an extra couple of minutes....open her orange juice without being a shithead about it.
I've been in hospitals where I knew and had dated some of the nurses. The Extra...is 5 extra people stop in when they're between tasks to say hi, ask if you need anything, or make some small talk to take your mind off of what is happening. Its very small things that make the difference. I doubt the guy was asking badgranny to dip into her oxycontin stash to share.
