Managerial stupidity cocked that up from the start.
Wrong pitcher was in to start the inning.
Infield should not have been brought in at that point.
If Adams plays back on the first grounder, the run scores, but there are two outs and the game is tied. Then the next grounder becomes a routine play to first and the inning is over.
That doesn't excuse his play, and he should have looked to third rather than throwing to second, but it certainly didn't help matters.
Situations where it is acceptable to bring in the infield, IMO:
8th inning or later of a tie game
6th inning or later of a one run game with the bases loaded.
It met neither of those criteria.
