Just finished the season finale. I'm loving it so far - the best Star Wars content Disney has produced outside of Rogue One, IMO.

Sabine gave it to Bo-Katan before the end of Rebels, at the conclusion of the storyline where she went back home to try to recruit clan Wren into the rebellion. So she didn't have it when she left with Ahsoka.

All us crazy people saying Dave Filoni was the best thing going in Star Wars for the last decade aren't looking so nuts now.

More than one flashback, of the same scene (Separatist droids attacking, he's rescued by Mandalorian's...).
